Vary for initial visit, full session, family session, and half sessions.
Reduced fee services are available on an individual basis.
Payment
Cash, check and all major credit cards accepted for payment.
Insurance
 Dr. Roth participates with Medicare, BCBS of Michigan, and Cofinity.
Services may be covered in full or in part by your health insurance or employee benefit plan.
The therapists at Commerce Psychiatric Services participate with additional insurances.
To determine if you have mental health coverage, the first thing you should do is check with your insurance carrier. Check your coverage carefully and find the answers to the following questions:
- What are my mental health benefits? What is my deductible?
- What is the coverage amount per therapy session?
- How many therapy sessions does my plan cover?
- How much does my insurance pay for an out-of-network provider?
- Is approval required from my primary care physician?
Confidentiality & Privacy Policy
The law protects the relationship between a client and a psychotherapist, and information cannot be disclosed without written permission.
Exceptions to confidentiality include:
 Suspected child abuse or dependant adult or elder abuse, for which I am required by law to report this to the appropriate authorities immediately.
If a client is threatening serious bodily harm to another person/s, I must notify the police and inform the intended victim.
If a client intends to harm him or herself, I will make every effort to enlist their cooperation in insuring their safety. If they do not cooperate, I will take further measures without their permission that are provided to me by law in order to ensure their safety. |
